What Happens After Your Initial Consultation?

After your consultation with Dr. Ortiz, you will have the option to choose a path based on your assessment and personal health needs
Return to Your Primary Care Provider

If continued rheumatology care is not required, you can return to your primary care provider at Paradigm Family Health or another primary care provider of your choice.

Ongoing Care with Paradigm + Connected Rheumatology

If a new rheumatic or autoimmune diagnosis is made, continued care with a rheumatologist will be recommended. You can choose to remain with Paradigm + Connected Rheumatology for ongoing rheumatic care or transition to another rheumatologist.

To ensure you receive the best ongoing care, we offer tailored membership plans allowing your primary care and rheumatology specialist care to coordinate together to optimize your health.
New Diagnosis Care Plan ($1299)
Continuous Care Plan ($999)
This plan provides dedicated support throughout that first year of a new diagnosis, offering personalized care and regular check-ins to optimize your treatment and enhance your well-being.
This plan ensures consistent follow-up and management of your condition, with regular visits to monitor your progress and make necessary adjustments for sustained health.

Frequently Asked Questions

Why should you get your rheumatic care with Paradigm + Connected Rheumatology?

Our healthcare system is hopelessly fragmented. Every specialist works in their own silos, never speaking to each other, instead communicating only through faxed medical records that have a lot of words but say very little.  We all suffer when our doctors don’t communicate but it is especially true when you have an autoimmune disease. Those with rheumatic or autoimmune disease need a TEAM to truly feel their best and that team starts with Primary Care and Rheumatology. Each brings their own expertise and you benefit.

Why do I need to commit to a yearly membership?

Autoimmune and rheumatic diseases unfortunately do not have a cure.  To provide the best care, we have designed a yearly membership plan that ensures appropriate access to your rheumatology specialist in addition to your primary care team.

What if I move or need to cancel my membership early?

We will gladly refund the prorated amount for your visits and can securely send your medical records to your next provider. However, please note that we will not be able to continue managing your rheumatological or autoimmune disease.

Can I see the rheumatologist and then switch all my care to just the primary care providers?

Unfortunately no. Rheumatological and autoimmune diseases are complex and often involve progression and regression. While your primary care provider at Paradigm Family Health with work in conjunction with your rheumatologist, they are not a substitute for a Rheumatologist and will not prescribe or manage these medications.
